Lunch and Learn Project Percolator with Maamwesying Ontario Health Team

This month, we are pleased to welcome spotlight presentations from our partners at Maamwesying Ontario Health Team. Over the past year, Maamwesying OHT has been actively working to improve the experience of patients, clients, caregivers, and providers navigating Transitions in Care. These transitions represent critical points in healthcare, where a person’s care and information are transferred between health and social service providers, interprofessional teams, and various care settings. Guided by the direct input and experiences of those involved in and impacted by these transitions, Maamwesying OHT has been implementing changes across the North East Region to create a more seamless, culturally grounded, and integrated care system.
